Without seeing the lesion, it is very difficult to assess what it is.
Skin lesions can be fungal in nature, or at least aggravated by a fungal condition.
But let's go with what your dermatologist has diagnosed you with. Prurigo nodularis is not a common condition.
In all my years of practice, I saw only one patient with this condition. His mother had it and his son had it. All three people (especially the patient himself, who had it the most severe), did very, very well "cleansing his body of toxins". His skin improved dramatically on a liver/metabolic cleanse.
I put him on a hypoallergenic diet and a metabolic liver cleanse.
It would definately be helpful to also get food allergy tested as certain foods for certain people can aggravate any skin condition.
Why are you anemic? If it is because of menstrual blood loss, then your liver is definately working overtime and a liver/metabolic cleanse would be very helpful for you. If you don't know why you're anemic, make sure you get your stools checked for occult blood to make sure the blood is not coming from your colon (i.e. you would need a colonoscopy if your blood loss was coming from your colon).
Mineral and vitamin imbalances can also aggravate skin conditions.
It's always best to see a licensed healthcare practitioner, however that's a start.
Yes, there is hope. I've seen it.
